A version of the Reform-Lisbon Treaty amending the Treaty on European Union and the Treaty establishing the European Community is available here. That looks like the version that is ready to be signed in Lisbon on December 13th, 2007.
The "Final Act" containing the various declarations and other bits and pieces is also available on the Council's website.
